Procedure for Requesting Accommodations

  1. Fill Out the Application

Register in the Accommodation Information Management (AIM) system. . (Despite the title, this form is also used for emotional support and service animal accommodations.)

  2. Upload Documentation

Communicate with Ken Clemens, the General Manager for Bon Appétit, about options for addressing dietary needs. Ken works with many students to identify options that meet their dietary needs. If that is not possible, he will communicate with DAS about Bon Appétit’s limitations. Contact Ken at kclemens@georgefox.edu or (503) 554-2500.

It is rare that Bon Appétit is unable to meet a student's needs and DAS approves a full exemption to the meal plan. In some instances a reduced meal plan is suitable. With that said, each application is looked at individually, recognizing unique circumstances for each student.

If, after communicating with Bon Appétit, a student wants to proceed with a meal plan accommodation request, they can upload documentation with their application or email it to das@georgefox.edu. NOTE: DAS cannot make a determination until documentation is submitted from an appropriate provider.

Ask your provider to complete the or write a letter to address your request. If your provider prefers to write a letter, the document should include the disability/medical/mental health diagnosis and detailed accommodation recommendations. Those recommendations should focus on necessary environmental or dietary needs, not specific locations. For example, a provider might indicate you need a location where you can access a kitchen, not that you need housing in specific campus apartments.
